C.H.E.K Level 3 - Writing Case Histories
 

To assist you with writing your case histories, here is a Q & A with Suzi Nevell, C.H.E.K Faculty and instructor for C.H.E.K Practitioner Level 3 Certification.

 

How many Case Histories do I need to write for C.H.E.K Level 3?

For anyone who is preparing to take the C.H.E.K Practitioner Level 3 Certification, you are required to submit up to three client case histories. 

 

How do I write a Case History? Is there a format I can follow?

The handout ‘Preparing Case Histories’ guides you in writing your Case Histories and outlines the format to follow.

 

I’ve created my own client evaluation forms, do I have to rewrite my file notes and use the CHEK Institute forms?

The Case History format in the guide may not match your own client evaluation forms. This format is just a guide for people to follow. You may submit any evaluation forms that you use in your practice.

 

Do I have to use the assessment forms that we were taught in each of the C.H.E.K Practitioner courses?

Yes. You must present ALL of the client testing that you were taught on the applicable assessment forms. You may scan these forms into the case history document as long as they are legible.

 

It is your choice as to how the information is presented as long as you complete the entire Level 1 and 2 assessments and document your client findings for all these tests.

 

There should also be a daily readiness form completed showing physiological load has been assessed.

 

The ONLY time some tests may be omitted is when there are pain and dysfunction present that prevent the test being performed safely as it may hurt the client. This must be documented in the Case History as to why you did not complete the test.

 

As the ‘Preparing Case Histories’ guides states, you are to present the initial evaluation, the initial phase of training with all programs, variables and periodisation completed in this document. All exercise programs must be completed in a typed document.

 

You are then required to present a follow-up re-evaluation and the program that followed the re-evaluation. All tests must be completed in the re-evaluation not just the tests the client failed in the initial assessment. You are to provide a summary of the client’s initial presentation and then a summary after your second program. You are to also document any other allied Health care providers you used as part of the total package to the client.

 

Can I write Case Histories after I complete C.H.E.K Level 1?

It helps to start assessing after completing the C.H.E.K Practitioner Level 1 Certification. Document all your findings and programs as you were taught, add the C.H.E.K Practitioner Level 2 Certification findings to the assessment and you are already halfway through the Case History.

 

How can I get a copy of the ‘Preparing Case Histories’ guide?

This is handed out to you at the C.H.E.K Practitioner Level 2 Certification class. You may request the ‘Preparing Case Histories’ guide after completing C.H.E.K Practitioner Level I Certification if you want to start earlier.  Email HQH to request a copy to be emailed to you.

 

Who do I send my Case Histories to and do I have to post them or can I email them?

Case Histories are sent to the C.H.E.K Institute. You are now able to email them to educate@chekinstitute.com.
